Transcutaneous electrical nerve stimulation (TENS) is a treatment option for patients suffering from chronic musculoskeletal pain. This therapy attaches low-voltage electrodes to a patient’s skin and delivers electrical impulses to the area causing pain. TENS may benefit those with muscle, joint or bone problems, as well as fibromyalgia, osteoarthritis, tendinitis or bursitis.

Although TENS is an effective therapy for some, it may not work for everybody. That’s when percutaneous electrical nerve stimulation (PENS) may be considered. Similar to TENS, PENS delivers low-voltage electrical currents to areas causing pain, but this treatment option uses needle electrodes inserted into the skin. This treatment option is usually reserved for patients who have not received adequate pain relief from TENS therapy.

During the procedure, the physician cleans and sanitizes the affected area. Unlike TENS, the physician will insert needle electrodes into the skin rather than placing a TENS electrode pad on top of the skin. Once the needles are in place and connected to the electrical stimulator, the device will be turned on and patients may experience muscle contractions.

Depending on the area being treated, PENS could take anywhere from 15 to 60 minutes to complete, and patients may require additional treatments. Although the risk of complication is minimal, patients could experience increased pain, bruising or bleeding.
